Summary
Clinical trial recruitment continues to fail at scale because conventional screening workflows cannot fully access eligibility relevant information recorded in narrative form, including disease stage, treatment response, biomarker results, and clinical reasoning, even when eligible patients are present in electronic health records. This narrative review examines applications of artificial intelligence, particularly large language models, across the recruitment pipeline. AI may improve candidate discovery and screening efficiency, but identifying a patient is not the same as enrolling a patient, and AI alone cannot remove the remaining human, logistical, ethical, and institutional barriers.
Key points
- The evidence is organized around two ceilings. The discovery ceiling concerns the ability to identify eligible candidates in clinical data, and current evidence suggests that AI can raise this ceiling and improve discovery and screening efficiency in defined workflows.
- The enrollment ceiling concerns the human, logistical, ethical, and institutional barriers that remain after a patient has been identified. Increased patient visibility does not ensure enrollment.
- Applications reviewed include trial design, predictive enrichment, site selection, and patient facing engagement. Governance is also required for equity, privacy, regulation, and workflow integration.
- Audience
- Clinicians, researchers, and advanced students interested in clinical trial recruitment, medical AI, clinical data science, and research governance
- Scope and limitations
- This is a narrative review, and its evidence base is described as concentrated in retrospective evaluations. Prospective, multicenter validation remains limited, and few studies have demonstrated improvements in enrollment, representativeness, or cost effectiveness. Recruitment AI should therefore be treated as a locally validated, human supervised support tool evaluated against downstream outcomes such as enrollment, diversity, burden, and cost.
- Research context
- This topic has a neutral thematic connection to Sangzin Ahn's portfolio through medical AI for unstructured clinical data, large language models, data driven clinical trial operations, and responsible evaluation of AI systems.
